Availability Referral/booking process - 1.Contact Reed Community Care on telephone 02088261949 with requirements of package. 2. 2. 'My Care Plan is sent out for completion. This will enable Reed to gain a full understanding of your requirements.(If assistance is required, Reed will provide support at your 1st Meeting). 3.3. 1st meeting arranged to conduct risk assessment, moving and handling assessment, medication requirements and sign paperwork and review 'My Care Plan'. The information is then taken back to the office for documents to be typed up. 4. Select appropriate care worker/s andinterview if required, then make final decision who you would like to provide support. 5. 2nd meeting arrange to place all completed paperwork is in file and ensure everything is in place for package to start.
